2. The patient can enter the registered phone number on patient's file with the clinic in the front desk login screen to start the check-in process.
3. If the patient’s entered phone number matches a registered record and an appointment is scheduled for the current date, the system will present the following forms:
Subjective
Pain Diagram (if configured)
Outcome Assessment Form (if configured)
Consent Forms (if not signed already)
Signature Page
Note:In the zHealth system, the patient must have an appointment scheduled as an existing patient and must have completed at least one check-in as a new patient.
Otherwise, the patient will continue to be prompted to complete the Social History, Surgical History, Medical History, and other intake forms until they are submitted once as a new patient.
