How to Change or Delete an ICD Code (that has been mapped)

ICD-10 codes are used to classify and code diagnoses, symptoms, and procedures. However, there may be instances where an ICD code needs to be changed or deleted, especially if it has been mapped incorrectly with the CPT codes. This guide will provide step-by-step instructions on how to change or delete an ICD code that has been mapped, ensuring your records remain accurate and up-to-date.

1. To delete or change an ICD code, open the invoice where you want to make the changes. While trying to delete an ICD code that is mapped to CPT code (check the Diagnosis Pointer in CPT code section), you will get a message saying "Diagnosis is already mapped." 

Diagnosis Pointers are used to describe relationships between submitted diagnoses and service line treatment information on claims and encounters.



2. Now, there are two ways to delete or change the ICD code that has been mapped. 

First Method: 

a. Add a new ICD code (for example, you added ICD code - M40. 4 Kyphossis Acquired Thoracic)
b. Suppose, you want to delete ICD code - M99. 00 Segmental and somatic dysfunction of head region. Use the "four arrow" icon as shown in the screenshot to move the this ICD code down.
c. Now, ICD code - M99.00 will no longer be mapped to the CPT code because the Diagnosis Pointer for this ICD code is "E". 
d. Now, go ahead and click the "Delete" icon for M99.00 to delete this ICD code. Click "Yes" in the pop-up window to proceed.






3. Second Method to Delete to Change mapped ICD code:

a. Go to the CPT code section. Suppose you want to delete the ICD code with "D" as Diagnosis Pointer.
b. In the CPT code's Diagnosis Pointer section, click the drop down menu for "D" and remove D from being mapped.
c. Click the "Save" icon in the CPT code line as shown in second screenshot.
d. Now, go ahead and click the "Delete" icon for the ICD code with "D" as Diagnosis Pointer. Click "Yes" on the pop-up window to continue.
