Step-by-Step Instructions: How to Download Patient Demographics
Use these guidelines to export your patient demographics to a CSV File from zHealthEHR.
1. Access your zHealth dashboard.
To search for all patients in your practice, click on the "Search" icon on the top navigation bar. |

2. You will see a list of all active and inactive patients at your practice.
|
|
|
3. Click on green button - "Download All Patients" - on the top right side of the screen. Save the file to your computer system.
Note: If you change the file name when saving, be sure to add .CSV to the end of the file name.

The exported file will include the following information:
- Patient ID Number
- MRN
- Patient First, Second, and Last Name
- Sex
- Birth Date
- Address
- Phone Number
- Email address
- Active/Inactive Status
- Primary Insurance ID
- Primary Insurance
- Secondary Insurance ID
- Secondary Insurance
- Referred By
- Employer
- Patient Type
