How to Setup Outcome Assessment Form


Patient-reported outcome assessments provide valuable insight into patient's well-being and facilitate better communication between providers and their patients. The Outcome Assessment (OA) form contains standardized, validated questions that help you measure the patients’ perception of their condition and the impact of chiropractic interventions on their health. zHealth has integrated the Outcome Assessment functionality within our software to help you collect the OA forms electronically, rather than on paper. You can configure the OA form for each patient by following the below steps:

1. Enter the first or last name of the patient in the Search box on the top of the navigation bar on the zHealth dashboard. Click Enter.



2. Click on the desired patient name from the auto-populated results. The Patient’s Chart opens.


3. By default, you will be at the Summary section of the patient profile.

On the right side of the screen, you will see a button called 'Configure O/A Form'. Click on that. A pop-up window opens.



4. Setup what outcome assessments you want the patient to complete and at what frequency by clicking the drop-down buttons.



5. Once you have made your selections, click on the Update button at the bottom of the pop-up screen.



6. Doing this will setup outcome assessment form for the patient and it will show up automatically. To view the OA form filled by the patient, click 'OA' tab as shown here.
