Patient Demographics Data


In this help guide, learn how to export your patient demographics to a CSV File from zHealth.

1. Access your zHealth dashboard. To search for all patients in your practice, click on the "Search" icon on the top navigation bar.



2. You will see a list of all active and inactive patients at your practice.



3. Click on green button - "Download All Patients" - on the top right side of the screen. You will receive a pop-up notification. "The Patient Data Export will be available in the Message Center in a few minutes."  




4. Go to the Message Center. The Patients Data is generated. Click the "Download" link and save* the file to your computer.
*Note: If you change the file name when saving, be sure to add .CSV to the end of the file name.



The exported file will include the following information:
  1. Patient ID Number
  2. MRN
  3. Patient First, Second, and Last Name
  4. Sex
  5. Birth Date
  6. Address
  7. Phone Number (this is the primary number listed in the patient chart)
  8. Email Address
  9. Active/Inactive Status 
  10. Primary Insurance ID
  11. Primary Insurance
  12. Secondary Insurance ID
  13. Secondary Insurance
  14. Referred By
  15. Employer
  16. Patient Type
