Review Family History, Medical History, Surgical History, and Social History Intake
zHealth's online patient intake offering providers an easy to use and intuitive experience for capturing patient medical, surgical, family, and social histories in addition to patient's initial complaint and signed consent forms.
zHealth sends the intake paperwork link to the patient prior to their appointment. Once the patient submits the intake form, the provider can check the information in the patient's profile. Consent forms, initial complaint forms, and histories are saved to the Intake section in the patient profile.

 1. Navigate to the Patient Chart. You will be on the Summary section of the Patient Chart. Click on the sixth tab - Intake. |
2. Under the Intake section, you will be able to view the first section - Family Medical History - by default. If the patient has submitted family history during intake, you can view the submitted information in this section. . |
3. Similarly, you can click on the other side panel options available on the left side of the screen to view Medical, Surgical, and Social histories. |
