This report provides details of the last appointment made by a specific patient at your practice.
Why you need it?
Use this report to know when a given patient visited your practice and if you should recall that patient. 
|  1. Access your zHealth account. On the Dashboard, you will see your name at the top right corner. Hover the cursor over to your name. A drop-down menu will open. Click on "Reports". | 
|  2. On the Reports window, scroll down to the Patient Reports section. Click on the Last Appointment Report option. A pop-up window will open. | 
|  3. Fill the following details to generate last appointment report:  a. Select the provider name. b. Enter patient's last and first name. c. Select date. d. Then click 'Generate Report'. | 
|  4. This is how the last appointment report will look like. | 
| Field Name | Description | 
| Patient Name | This is the name of the patient | 
| Phone | This is the first contact number of the patient which is there in Contact Details in the Patient Profile | 
| This is the email address of the patient | |
| Created Date | This is the date when the first SOAP note was created for the patient visit | 
| Last Appointment Date | This field indicates the date and time when the patient last visited your practice |
