Sign a SOAP Note

Providers must review and sign a note. Signing a note is part of the electronic charting workflow and affects how chart entries are finished. Signing a SOAP note indicates that a note is complete and no more changes should be made to it.

1. Navigate to Patient Chart / Notes

Enter the first or last name of the patient in the top navigation bar search box. Click to select the patient from the auto-populated results. The Patient's Chart Face page opens. 

The first section in the
Summary page shows all editable notes that yet to be signed by the provider.

In the Notes list, click
Edit on the note you want to sign. The Note opens. 



2. Review and Sign Note

Click Sign Note on the bottom Notes bar to finalize the note and change it to a Signed status. Notes can only be signed by Providers. Once a note has been signed, no additional changes can be made.
